The FDA posted a potentially high-risk alert for a heart pump controller this week.

The urgent lesson reaches beyond one hospital device: when a machine supports life, the backup must be part of the system, not an afterthought.

The Current Signal

On August 26, FDA said Abiomed was removing Automated Impella Controllers because of a purge cassette recognition issue.

The controller runs and watches an Impella heart pump. A small purge flag helps the controller know that a purge disc is in place.

If that part fails, the controller may show a “Purge Disc Disconnected” or “Purge Disc not Detected” alarm. FDA says trained staff should follow the device instructions, try reinserting the disc, and switch to a backup controller if the alarm does not clear.

As of August 17, Abiomed had reported 37 serious injuries and three deaths tied to the issue. FDA was still reviewing it.

This is mainly a hospital action. Patients should not handle hospital equipment or change care on their own. But families can still learn one useful rule: know the name of a critical device and know who owns the backup plan.

U.S. Parallel: Apollo 13, April 1970

Apollo 13 survived because the life-support chain and available parts were made visible.

On April 13, 1970, an oxygen tank exploded aboard Apollo 13. The moon landing ended at once. The job became bringing Jim Lovell, Jack Swigert, and Fred Haise home.

The crew moved into the lunar module and used it as a lifeboat. Then carbon dioxide began to rise.

NASA had enough lithium hydroxide canisters to remove the gas. But the command module used square canisters, and the lunar module had round openings.

Mission Control had to know every item available on the spacecraft. Engineers built a filter adapter from the same kinds of parts the crew had, then read clear steps up to space.

The story is famous as a “square peg in a round hole.” The deeper lesson is about the chain. Oxygen, power, water, filters, parts, people, and instructions all had to connect.

A backup was not useful because it existed somewhere. It became useful when its name, location, fit, and next action were known.

A hospital device alert is not a space mission. The narrow link is life support. In both cases, a small component can create a large risk, and the response gets faster when the system is visible.

Ancient Parallel: Persia Built Access For Repair

Ancient Persian water systems stayed useful because workers could reach and repair the hidden chain.

Across ancient Persia, towns in dry land depended on qanats. These were gently sloped tunnels that moved groundwater downhill by gravity.

The main tunnel could run for many miles. A deep “mother well” reached the water source. Then workers dug vertical shafts at set points along the route.

Those shafts did more than remove soil. UNESCO explains that they brought air and gave workers access for repair and maintenance.

That was the smart part. The builders did not hide the whole life-support system underground and hope it would never fail. They built ways to reach the hidden links.

Communities also managed how the water was shared. The tunnel, shafts, workers, rules, and users formed one system.

Modern medical devices are vastly different. A qanat cannot teach someone how to run a heart pump. The useful parallel is design: critical systems need named access points, trained people, and a known backup.

At home, the “access shaft” is often a card. It tells a spouse, adult child, or caregiver what the device is, which alarm matters, who to call, and where the official instructions live.

The Pattern To Notice

Across both examples, the pattern is this: a life-support system is safer when its hidden links can be named, reached, and backed up.

Do not try to become the technician. Make the technician, device, alarm, and backup easy to find.

  1. Write the exact device name and model from the label or patient card.

  2. Write the exact alarm words that require a call. Use only the official guide.

  3. Write the first approved action. Do not invent one.

  4. Write the backup contact: clinic, device team, home-health team, or emergency number.

  5. Put one copy by the device and one in the caregiver’s phone.

  6. Ask the care team to check the card at the next visit.

Measured win: another adult can read all four lines in under one minute.

Status Check

  • Green: Device, alarm, action, and contact are all clear.

  • Yellow: One line is based on memory. Verify it.

  • Red: Nobody can name the device or the trained backup contact.
